ABSTRACT

Objective:To explore the effect of group visits on health condition among follow-up patients with chronic heart failure.Methods:Totally 126 outpatient follow-up patients with heart failure were divided into intervention group and control group by random number table from 2018 to 2019. The intervention group consisted of 63 cases and control group consisted of 59 cases. The intervention lasted 6 months. The intervention group received group visits, while the control group received routine outpatient follow-up. Medication adherence, quality of life and heart failure related indicators were evaluated at baseline and after 6 months of intervention.Results:At 6 months after intervention, the scores of medication adherence, total quality of life, body, emotion and others dimensions of the intervention group were (5.79±1.38), (30.11±8.22), (12.65±5.53), (5.24±4.57) and (12.22±4.76) points. These scores of the control group were (5.31±1.09), (37.26±9.02), (15.87±5.21), (7.03±5.14), (14.36±5.54) points. The differences between the two groups were statistically significant ( t values were -4.581-2.161, P<0.05 or 0.01). The BNP and proportion of New York Heart Association (NYHA) I the intervention group were (180.87±174.92) ng/L and 84.1% (53/63). These indicators of the control group were (351.02±268.13) ng/L and 67.8% (40/59). The differences between the two groups were statistically significant ( t value was -4.177, χ2 value was 4.484, P<0.01 or 0.05). Conclusions:Group visits program is an effective management mode to provide intensive patient education and foster peer support, improving medication adherence and quality of life of follow-up patients with heart failure.

ABSTRACT

Objective To investigate the prevalence of depression in Parkinson's disease ( PD) patients, analyze the clinical features of depression in PD patients , and evaluate its impact on quality of life. Methods One hundred and ninety-five PD patients and 63 normol controls were recruited in this study.The detailed clinical information was documented.Unified Parkinson's Disease Rating Scale and Hoehn-Yahr stage were used to evaluate the severity of motor function impairment in PD patients.Hamilton Depression Rating Scale (HAMD) and Hamilton Anxiety Rating Scale were employed to assess the severity of depression and anxiety in PD patients.The 39-item Parkinson's Disease Questionnaire was applied to assess the quality of life.The cross-sectional data were calculated with SPSS 21.0 statistic software, and P <0.05 was considered statistically significant.Results The average score of HAMD was 8.74 ±5.51 in 195 PD patients.Depressive symptoms were found in 54.4%of the PD patients ( mild depression 48.7% and moderate depression 5.6%).Depression significantly impaired the quality of life in PD.Compared with PD without depression, PD with depression earned more scores in anxiety factor (4 (2, 5) vs 1(0, 2), Z= -8.69, P=0.00), blocker factor (2 (1, 3) vs 0(0, 1), Z=-7.95, P=0.00), cognitive factor (1 (0, 2) vs 0(0, 0), Z=-7.01, P=0.00), sleep factor (2(1, 3) vs 0(0, 1), Z=-6.42, P=0.00) and despair factor (2 (1, 3) vs 1 (0, 1), Z=-7.16, P=0.00).There was no significant difference in day and night change (0(0, 0) vs 0(0, 0), Z=-0.19, P=0.85) and body weight (0(0, 0) vs 0(0, 0), Z=-1.28, P=0.20) between these two groups.The PD with depression obtained higher scores in total quality of life (30(22, 44) vs 14 (5, 24), Z=-7.03, P=0.00), motor function (6 (2, 13) vs 1 (0, 5), Z=-3.67, P=0.00), daily life ability (4 (1, 8) vs 1 (0, 4), Z=-2.81, P=0.01) , emotional health (5 (2, 11) vs 0 (0, 2), Z=-5.82, P=0.00), humiliation (2 (0, 5) vs 0 (0, 1), Z=-3.10, P=0.00), social support (0 (0, 1) vs 0 (0, 0), Z=-2.86, P=0.00), recognition function (4 (2, 6) vs 2 (0, 4), Z=-2.87, P=0.00), sociability(1(0, 3) vs 0(0, 1), Z=-3.25, P=0.00), and body pain (3 (1, 6) vs 1 (0, 2), Z=-3.91, P=0.00) than patients without depression.Conclusions Incidence of depression ( mainly mild ) in PD patients is high. Depressive symptoms significantly affect the quality of life of PD patients.

ABSTRACT

Objective To observe the clinical curative effect of zhidanhuayu decoction on patients with carotid atherosclerotic plaque.Methods 80 patients with carotid atherosclerosis were randomly divided into two groups, each group of 40 cases, the control group was treated with rosuvastatin calcium 20 mg, once every night.The treatment group in the control group on the basis of a scheme based on the use of night zhidanhuayu decoction water 500 mL temperature and heat to 200 mL, then add water and cook until 200 mL to 100 mL, on the afternoon of two phase decoction against half amount of clothes, a daily agent;the two groups were 3 months for 1 courses.Before and after treatment, the mean medial thickness(IMT), plaque score, blood lipid and adverse reactions were observed.Results There was significant difference between the two groups before and after treatment, the difference was significant between the medial and middle thickness(IMT), plaque score and blood lipid(P< 0.05).After treatment, compared with the control group, the treatment group and the control group, IMT, plaque score, blood lipids were statistically significant(P< 0.05).Conclusion Zhidanhuayu decoction can adjust the blood lipids, but also has the effect of anti atherosclerosis.

ABSTRACT

BACKGROUND: The choline acetyltransferase (CHAT) is the key synthetic enzyme for acetylcholine, and is the important symbol of the functional activity of the cholinergic system. There was a relationship of the cholinergic neurons damage with the pathogenesis of Alzheimer disease (AD) and the mild cognitive impairment (MCI). Whether or not the fimbria/fornix transection may affect the expressive variety of ChAT in different cerebral regions of rats (cortex, hippocammpi CA1, amygdala, Meynert nucleus) is very important for the recognition of the pathogenesis of AD and MCI and the establishment of experimental animal model of AD.OBJECTIVE: To observe the expressive variety of ChAT in different cerebral regions of rats with fimbria/fornix transection and discuss exploratorily the methods of simulative experimental AD.DESIGN: A randomized and control study.SETTING: Institute of Cerebrovascular Disease, Affiliated Hospital of the Qingdao University Medical College and the Department of Neurology, No. 1People' s Hospital of Jining.MATERIALS: The experiment was completed in the Institute of Cerebrovascular Disease, Affiliated Hospital of the Qingdao University Medical College from March to December 2003. Totally 14 adult healthy female Wistar rats aged 5 months were randomly divided into model group and control group with 7 in each group.METHODS: ① The bilateral fimbria-fornix of brain in model group were transected on the stereotaxic apparatus to set animal model. After opening the skull, at the coordinates, along the bregma posterior 2.2 mm-2.5 mm and lateral 1.0 mm according to the atlas of Paxinos and Watson, and the dorsal fornix, the anterior part of hippocampus and the fimbria were cut off with a double blade under visual inspection. Rats in the control group were not performed with fimbria/fornix transection, and the other procedures were completed as those in the model group. ② On the 28 day after the surgery, all rats were killed under anesthesia to take out the brain tissues and make coronary sections for histochemical observation in a immunohistochemical way. The expressions of the ChAT positive neurons were observed in the cortex, hippocammpi CA1, amygdala, Meynert nucleus, and the brown neurons under microscopy was the ChAT positive neurons.MAIN OUTCOME MEASURES: Expressions of ChAT positive neurons in the cortex, hippocammpi CA1, amygdala, Meynert of the basal forebrain in the model group and the control group.RESULTS: All the rats entered the final analysis without any loss. The expression of ChAT positive neurons in the cortex, hippocammpi CA1,amygdala, Meynert of the basal forebrain in the model group were distinctly decreased than that in the control group (2.97±1.45, 32.60±7.33, t=10.51,P < 0.01); (6.83±2.41, 50.57±5.85, t=1 8.30, P< 0.01); (14.43±6.75, 35.43±10.49,t=4.47, P < 0.01); (5.77±6.62, 48.77±7.10, t=1 1.72, P < 0.01), and the differences were significant.CONCLUSION: Finbria/fomix transection can decrease the expression of ChAT positive neurons in many cerebral regions of rats, and can be used in a method of setting the experimental animal model of AD.
